Restorative proctocolectomy with ileal pouch-anal anastomosis (RP/IPAA) is the procedure of choice for patients with ulcerative colitis (UC) requiring surgery (Fig. 1). 1,2 RP/IPAA consists of a J, S, or W ileal reservoir anastomosed either by a stapled or hand-sewn technique.

Methods: Twenty-four patients underwent ileal pouch-anal anastomosis for ulcerative colitis. Eleven were randomly assigned to the J-pouch group and 13 to the W-pouch group. Frequency of defecation and other functional data were collected at 4, 8, and 12 months after ileostomy closure.

The use of a J pouch was associated with a significantly higher prevalence of use of anti-diarrhoeal medication (J vs S: OR 2.80, P = 0.01; J vs W: OR 3.55, P < 0.001). Conclusion: All three reservoirs had similar perioperative complication rates.

I know that the VAST majority of surgeries eventually end with a J-pouch (small intestine being folded into a J shape.) But has anyone here had a W pouch performed, or going to get one made?
