We report a case of cytomegalovirus- (CMV-) induced buccal ulcer in a patient with nephrotic syndrome. An 82-year-old man with membranous nephropathy was on immunosuppressive therapy presented with an ulcer in the oral cavity and was hospitalized.

Cytomegalovirus (CMV) is another member of the human herpesvirus family that uncommonly presents with oral ulcerations. These non-specific ulcerations occur in immunosuppressed patients, and most commonly affect the tongue, floor of mouth, and hard or soft palate [ 27 , 28 ].

In this article a case of CMV infection associated with oral mucosal ulceration and a brief review of the subject is presented. Establishing the cause of ulceration is important in determining a definitive diagnosis and prescribing appropriate therapy.

In such immunosuppressed patients, CMV and HSV-1 may coexist and produce painful ulcerations on the edge of the tongue, lips, and mucous membranes. CMV ulcerations are nonspecific and occur more frequently in patients with AIDS that usually present with keratotic skin lesions and mucosal/skin ulcerations.

The most common manifestation of cytomegalovirus infection of the gastrointestinal tract including the oral mucosa is ulceration. The role of cytomegalovirus in xerostomia, Sjögren's syndrome, and Kaposi's sarcoma is continuing to be investigated.

CMV infection, which is common in renal transplant recipients, only rarely affects the mouth. Herein we have reported 6 patients who suffered oral ulcers due to CMV infection. An early diagnosis of these lesions is important to a successful outcome for these patients.
