Surgical decompression included anterior, minimus, and middle scalenectomy and brachial plexus neurolysis via a supraclavicular incision and pectoralis minor tenotomy through a small vertical infraclavicular incision. No first ribs were excised.

The technique of an operation, complete anterior scalenectomy, specifically designed to relieve the upper plexus type of TOS is presented, along with the congenital anomalies that invariably seem to be associated with this syndrome.
